Requests For Memorial Certificates

Family and friends of a deceased veteran or someone acting on their behalf may request a Presidential Certificate a request form (va-400-0247) is available on the National Cemetery Administration Website www.cem.va.gov. 

A military Discharge document must be submitted with the request.
